Dr Nayan Brahme’s Groundbreaking TB Research
A Closer Look at Tuberculosis in HIV Patients
👨⚕️ Who Is Dr. Nayan Brahme?
Dr. Nayan Brahme is a Senior Medical Doctor practicing at Aarogyaclinics and earlier associated with Lokmanya Tilak Municipal Medical College (LTMMC), Sion, Mumbai, specializing in internal medicine and infectious diseases. His acclaimed work on tuberculosis among HIV-positive vs. HIV-negative patients offers invaluable clinical insights.
🔬 Landmark Study on TB-HIV Co-Infection
In collaboration with the School of Tropical Medicine, Kolkata, Dr. Brahme conducted a comparative clinico-microbiological study titled Comparative Study of Tuberculosis in HIV-Positive and HIV-Negative Patients from July 2013 to June 2014. The study included 50 participants—25 HIV-positive and 25 HIV-negative.
🧪 Key Findings
- TB Manifestation: HIV-negative patients predominantly showed pleural effusion; HIV-positive patients showed more cervical lymphadenopathy and disseminated TB.
- Symptoms: Fever was common (90%) in both. Cough was more frequent in HIV-negative, while abdominal pain and lymph node swelling occurred more in HIV-positive.
- Drug Resistance: No significant difference in INH/RIF resistance; 18% overall resistance across both groups.
- CD4 Correlation: Most HIV-positive patients had CD4 <200, with disseminated TB significantly higher in this subgroup.
- Radiology: HIV-negative patients showed more abnormal chest X-rays; HIV-positive patients more often had normal X-rays, which can delay diagnosis.
🌍 Clinical & Public Health Relevance
These findings highlight the need for tailored TB screening and diagnostic protocols in HIV-positive individuals, particularly when chest X-rays appear normal. They also support immune-status–phased TB treatment strategies and reinforce focused outreach to high-risk populations.
